| 目的:观察火针阿是穴联合刺络拔罐对带状疱疹后神经痛(postherpetic neuralgia,PHN)患者疼痛改善及预后的影响。方法:选取PHN患者70例,将其随机分为治疗组与对照组,每组各35例。对照组予以西医常规药物治疗,治疗组在对照组基础上予以火针阿是穴联合刺络拔罐治疗,2组均连续治疗4周。比较2组治疗前后疼痛视觉模拟评分法(Visual Analogue Scale,VAS)评分、疼痛介质指标[神经降压肽(neurotensin,NT)、β-内啡肽(β-endorphine,β-EP)、P物质(substance P,SP)]水平,评价疗效并记录患者预后情况。结果:总有效率治疗组为88.57%(31/35),对照组为68.57%(24/35),2组比较,差异有统计学意义(P<0.05)。2组VAS评分、疼痛介质指标治疗前后组内比较及治疗后组间比较,差异均有统计学意义(P<0.05)。疼痛复发率治疗组为5.71%(2/35),对照组为22.86%(8/35),2组比较,差异有统计学意义(P<0.05)。结论:火针阿是穴联合刺络拔罐治疗PHN疗效确切,可有效缓解患者疼痛症状、调节疼痛介质水平,并降低疼痛复发率,改善预后,值得临床推广。 |

| Clinical effect of fire needle at Ashi point combined with blood-letting puncture and cupping in treatment of postherpetic neuralgia: An analysis of 35 cases |

| Objective: To investigate the clinical effect of fire needle at Ashi point combined with blood-letting puncture and cupping in the treatment of postherpetic neuralgia (PHN). Methods: A total of 70 patients with PHN were randomly divided into treatment group and control group, with 35 patients in each group. The patients in the control group received conventional Western medicine treatment, and those in the treatment group received fire needle at Ashi point combined with blood-letting puncture and cupping in addition to the treatment in the control group. Both groups were treated for 4 consecutive weeks. The two groups were compared in terms of Visual Analogue Scale (VAS) score and pain mediators (neurotensin, β-endorphine, and substance P). Clinical outcome was assessed, and the prognosis of patients was recorded. Results: There was a significant difference in overall response rate between the treatment group and the control group [88.57% (31/35) vs 68.57% (24/35), P < 0.05]. After treatment, both groups had significant changes in VAS score and pain mediators, and there were significant differences in these indicators between the two groups after treatment (P < 0.05). There was a significant difference in pain recurrence rate between the treatment group and the control group [5.71% (2/35) vs 22.86% (8/35), P < 0.05]. Conclusion: Fire needle at Ashi point combined with blood-letting puncture and cupping has a marked clinical effect in the treatment of PHN and can effectively alleviate pain, regulate the levels of pain mediators, reduce pain recurrence rate, and improve prognosis. |
