国医大师孙光荣基于“升降相因”理论治疗眩晕的学术经验

摘要:
介绍国医大师孙光荣教授(以下尊称“孙老”)基于“升降相因”理论治疗眩晕的学术思想与临床经验。孙老立足经典,融汇诸家,以 “中和” 思想为指导,基于“升降相因”理论,强调气机升降动态平衡是人体生理常态,眩晕之病机多责之于气机升降失常。临证立足脾、肝、肾三脏,脾虚气陷则清阳不升,肝气郁结则气逆于上,肾精亏虚则升降无根,三者皆可导致清气不升、浊阴上犯,清窍失养或被浊邪扰动,发为眩晕。孙老临证以“升降相因”为纲,注重调节气机升降动态平衡,或健脾以复中枢,或调肝以畅疏泄,或益肾以固根基,清阳得升,浊阴得降,神明清静,则眩晕自止。文末附典型验案 1 则,以印证其理论与治法。

Academic experience of National Chinese Medicine Master Sun Guangrong in treatment of vertigo based on the theory of “interdependence between ascending and descending”

Abstract:
This article introduces the academic thinking and clinical experience of National Chinese Medicine Master Sun Guangrong in the treatment of vertigo based on the theory of “interdependence between ascending and descending”. Rooted in the classics and integrating insights from various schools, Professor Sun emphasizes that the dynamic balance of Qi movement (ascending and descending) constitutes the physiological norm of the human body based on the philosophy of “balance and harmony” and the theory of “interdependence between ascending and descending”, and the pathogenesis of vertigo is largely attributed to the disorder of this Qi movement. In clinical practice, the treatment focuses on the spleen, liver, and kidney; spleen deficiency and Qi collapse can lead to the failure of the ascending of clear Yang, stagnation of liver Qi can result in the reversed flow of Qi with upward rebellion, and deficiency of kidney-essence may result in a lack of support for ascending and descending; all three conditions can cause the failure of the ascending of clear Qi and the upward invasion of turbid Yin, thereby causing the clear orifices to be deprived of nourishment or disturbed by turbid pathogens and thus triggering vertigo. Professor Sun adopts the theory of “interdependence between ascending and descending” as the guiding principle in clinical practice and focuses on regulating the dynamic balance of Qi movement, with the methods of invigorating the spleen to restore the central nervous system, regulating the liver to smooth the free flow of Qi, and tonifying the kidney to consolidate the foundation. Vertigo naturally ceases ehen clear Yang ascends, turbid Yin descends, and the spirit remains clear and calm. A typical proven case is attached at the end of the article to verify his theory and treatment methods.
