国医大师何成瑶运用“养肝血、扶肝阳”法辨治痛经经验

摘要:
总结国医大师何成瑶教授(以下尊称“何老”)运用“养肝血、扶肝阳”法辨治痛经的学术思想与临床经验。何老认为,痛经的核心病机为肝血亏虚、经血乏源,疏泄失职、瘀滞胞宫,阴损及阳、肝阳不足,其治疗紧扣 “肝体阴而用阳” 理论,提出以养肝血为根本、以扶肝阳为关键、以通胞宫为目的,遵循 “养血疏肝以生气、活血化瘀通胞宫,养血补肝以固本、补肾温阳以调经,血足阳自生、阳煦血自行” 的思路,强调补血为主、温阳为辅,阴中求阳,忌峻补温燥。临床肝、脾、肾同调,使肝血充、肝阳复、气机畅、瘀血去,则痛经自止。文末附验案 1 则,佐证其理论与治法,为中医药辨治痛经提供思路与实践参考。

Experience of National Chinese Medicine Master He Chengyao in treatment of dysmenorrhea using the method of “nourishing liver blood and supporting liver Yang”

Abstract:
This article summarizes the academic thinking and clinical experience of National Chinese Medicine Master Professor He Chengyao in the syndrome differentiation-based treatment of dysmenorrhea using the method of “liver-blood supporting Yang”. Professor He believes that the core pathogenesis of dysmenorrhea includes deficiency of liver blood leading to a lack of source for menstrual blood, dysfunction of the liver in governing free flow leading to stagnation in the uterus, and Yin deficiency affecting Yang resulting in insufficiency of liver Yang. Professor He adheres to the theory of “Liver belonging to Yin in physique and Yang in function” and proposes a treatment approach that takes nourishing liver blood as the foundation, supporting liver Yang as the key, and unblocking the uterus as the goal. She follows the thinking of “nourishing the blood and soothing the liver to generate Qi, activating blood and resolving stasis to unblock the uterus, nourishing the blood and tonifying the liver to consolidate the constitution, and tonifying the kidney and warming Yang to regulate menstruation”, as well as the theory of “sufficient blood leads to spontaneous generation of Yang and warmed Yang ensures smooth circulation of blood”, and she emphasizes the priority of blood tonification with warming Yang as a supplement, the importance of reinforcing Yang from Yin, and the avoidance of drastic tonification and harsh warming dryness. In clinical practice, she coordinates the regulation of the liver, spleen, and kidney to ensure sufficient liver blood, restore liver Yang, smooth the flow of Qi, and remove blood stasis, and thus dysmenorrhea stops naturally. A proven case is attached at the end of the article to support her theory and treatment methods, so as to provide ideas and a practical reference for the traditional Chinese medicine syndrome differentiation-based treatment of dysmenorrhea.
