| 系统梳理近年来相关研究,总结中药单体及复方通过调控巨噬细胞极化干预糖尿病视网膜病变(diabetic retinopathy,DR)的研究进展。其核心机制主要涉及核因子κB(nuclear factor kappa-light-chain-enhancer of activated B cells,NF-κB)和硫氧还蛋白相互作用蛋白(thioredoxin-interacting protein,TXNIP)/热蛋白结构域相关蛋白3(NOD-like receptor thermal protein domain associated protein 3,NLRP3)等关键信号通路,中药单体及中成药、中药复方可通过抑制M1型巨噬细胞活化、促进M2型极化,纠正免疫微环境失衡,减轻视网膜炎症,保护血-视网膜屏障,延缓微血管病变进展。目前该领域仍存在机制研究不深入、实验模型单一、复方药效成分解析不足、临床转化困难等问题,未来需加强多学科交叉,深化机制验证,优化实验模型与临床试验设计,推动中医药防治DR的成果转化。 |

| Research advances in monomers and compound prescriptions of Chinese herbs in treatment of diabetic retinopathy based on macrophage polarization |

| This article systematically reviews related studies in recent years to summarize the research advances in monomers and compound prescriptions of Chinese herbs in the treatment of diabetic retinopathy. The core mechanisms mainly involve the key signaling pathways such as the nuclear factor-kappa B signaling pathway and the thioredoxin-interacting protein/NOD-like receptor protein 3 signaling pathway. Monomers of Chinese herbs, Chinese patent drugs, and compound prescriptions can inhibit M1 macrophage activation, promote M2 polarization, correct the imbalance of the immune microenvironment, alleviate retinal inflammation, protect the blood-retinal barrier, and delay the progression of microangiopathy. At present, there are still certain challenges in this field, including insufficient depth in mechanistic research, limited experimental models, inadequate analysis of active components in compounds, and difficulties in clinical translation, and in the future, it is recommended to strengthen multidisciplinary integration, deepen mechanistic verification, optimize experimental models and clinical trial designs, and promote the transformation of achievements in the prevention and treatment of DR by traditional Chinese medicine. |
