| 目的：观察针刺联合参芪活血汤治疗糖尿病下肢血管病变（diabetic lower extremity vascular disease，DLEVD）的临床疗效，及其对下肢血流量、踝肱指数（ankle brachial index，ABI）、炎症因子、血管内皮功能和氧化应激指标的影响。方法：将89例DLEVD患者随机分为治疗组和对照组，其中治疗组45例（80条患肢），对照组44例（80条患肢）。对照组予以糖尿病常规治疗及针刺治疗，治疗组在对照组基础上加用参芪活血汤治疗，2组均连续治疗8周。比较2组患者治疗前后的下肢血流量、ABI、炎症因子［白细胞介素-6（interleukin-6，IL-6）、肿瘤坏死因子 -α（tumor necrosis factor-α，TNF-α）］、血管内皮功能指标［内皮素-1（endothelin-1，ET-1）、一氧化氮（nitric oxide，NO）］及氧化应激指标［超氧化物歧化酶（superoxide dismutase，SOD）、丙二醛（malondialdehyde，MDA）］水平。结果：治疗后，2组患者下肢血流量、ABI、IL-6、TNF-α、ET-1、NO、SOD、MDA治疗前后组内比较及治疗后组间比较，差异均有统计学意义（P<0.05）。结论：针刺联合参芪活血汤可显著改善DLEVD患者下肢血流量和ABI，有效调节血管内皮功能和氧化应激指标，临床疗效良好，值得临床推广。 |

| Clinical effect of acupuncture combined with Shenqi Huoxue Decoction in treatment of diabetic lower extremity vascular disease: An analysis of 45 cases |

| Objective: To investigate the clinical effect of acupuncture combined with Shenqi Huoxue Decoction in the treatment of diabetic lower extremity vascular disease （DLEVD） and its impact on lower extremity blood flow volume，ankle-brachial index （ABI），inflammatory factors，vascular endothelial function，and oxidative stress parameters. Methods: A total of 89 patients with DLEVD were randomly divided into treatment group with 45 patients （80 affected limbs） and control group with 44 patients （80 affected limbs）. The patients in the control group were given conventional treatment of diabetes and acupuncture，while those in the treatment group were given Shenqi Huoxue Decoction in addition to the treatment in the control group，and the course of treatment was 8 consecutive weeks for both groups. The two groups were compared in terms of lower extremity blood flow volume，ABI，inflammatory factors （interleukin-6 ［IL-6］ and tumor necrosis factor-α ［TNF-α］），vascular endothelial function parameters （endothelin-1 ［ET-1］ and nitric oxide ［NO］），and oxidative stress parameters （superoxide dismutase ［SOD］ and malondialdehyde ［MDA］） before and after treatment. Results: After treatment，both groups had significant changes in lower extremity blood flow volume，ABI，IL-6，TNF-α，ET-1，NO，SOD，and MDA，and there were significant differences in these indicators between the two groups after treatment （P < 0.05）. Conclusion: Shenqi Huoxue Decoction has a marked clinical effect in patients with DLEVD and can significantly improve lower extremity blood flow volume and ABI and effectively regulate vascular endothelial function and oxidative stress parameters. |
