| 目的：观察参乌益智胶囊联合认知行为疗法（cognitive behavioral therapy，CBT）治疗卒中后认知障碍（post-stroke cognitive impairment，PSCI）的疗效及安全性。方法：选取PSCI患者90 例，将其随机分为治疗组和对照组，每组各45 例。对照组予以脑卒中基础治疗和认知行为疗法，治疗组在对照组基础上加用参乌益智胶囊治疗，2组均治疗28 d。比较2组治疗前后简易智力状态检查量表（Mini-Mental State Examination，MMSE）评分、蒙特利尔认知评估量表（Montreal Cognitive Assessment，MoCA）评分、中医证候积分、日常生活活动能力评价（Activity of Daily Living Scale，ADL)量表评分和肿瘤坏死因子-α（tumor necrosis factor-α，TNF-α）、白细胞介素-6（interleukin-6，IL-6）、同型半胱氨酸（homocysteine，Hcy）水平，并评价临床疗效。结果：总有效率治疗组为82.22%（37/45），对照组为60.00%（27/45），2组比较，差异有统计学意义（P＜0.05）。治疗后2组MMSE、MoCA、ADL评分均较治疗前升高（P＜0.05），且治疗组各项评分均高于对照组（P＜0.05）；2组TNF-α、IL-6、Hcy、中医证候积分均较治疗前降低（P＜0.05），且治疗组各项指标均低于对照组（P＜0.05）。结论：参乌益智胶囊联合认知行为疗法能有效提高PSCI患者的认知功能，降低炎症反应，改善日常生活能力。 |

| Clinical effect of Shenwu Yizhi Capsule combined with cognitive behavioral therapy in treatment of post-stroke cognitive impairment：An analysis of 45 cases |

| Objective：To investigate the clinical effect and safety of Shenwu Yizhi Capsule combined with cognitive behavioral therapy （CBT) in the treatment of post-stroke cognitive impairment （PSCI).Methods：A total of 90 patients with PSCI were enrolled and randomly divided into treatment group and control group，with 45 patients in each group.The patients in the control group were given the basic treatment for stroke and CBT，while those in the treatment group were given Shenwu Yizhi Capsule in addition to the treatment in the control group；the course of treatment was 28 days for both groups.The two groups were compared in terms of Mini-Mental State Examination （MMSE) score，Montreal Cognitive Assessment （MoCA) score，traditional Chinese medicine （TCM) syndrome score，Activities of Daily Living （ADL) score，and the levels of tumor necrosis factor-α （TNF-α)，interleukin-6 （IL-6)，and homocysteine （Hcy) before and after treatment，and clinical outcome was assessed.Results：There was a significant difference in overall response rate between the treatment group and the control group［82.22% （37/45) vs 60.00% （27/45)，P < 0.05］.After treatment，both groups had significant increases in MMSE，MoCA，and ADL scores （P < 0.05)，and the treatment group had significantly higher scores than the control group（P < 0.05)；both groups had significant reductions in TNF-α，IL-6，Hcy，and TCM syndrome score （P < 0.05)，and the treatment group had significantly lower values than the control group （P < 0.05).Conclusion：For patients with PSCI，Shenwu Yizhi Capsule combined with CBT can effectively improve cognitive function，alleviate inflammatory response，and improve activities of daily living. |
