| 目的：观察穴位注射正清风痛宁治疗滑膜肿胀型膝骨关节炎（knee osteoarthritis， KOA）寒湿
痹阻证的临床疗效。方法：选取滑膜肿胀型KOA 寒湿痹阻证60 例，将其随机分为治疗组和对照组，每组各
30 例。以4 周为1 个疗程，对照组口服双醋瑞因和塞来昔布，治疗组在对照组基础上加用穴位注射正清风
痛宁，治疗过程中2 组各脱落患者2 例。比较2 组治疗前，治疗后1 周、2 周、4 周的视觉模拟量表（Visual
Analogue Scale，VAS）评分、膝关节量表（Lysholm Knee Scoring Scale，Lysholm）评分及膝关节液中白细胞
介素-1β（interleukin - 1β，IL-1β）、肿瘤坏死因子-α（tumor necrosis factor-α，TNF-α）水平，并评
价临床疗效。结果：总有效率治疗组为100%（28/28），对照组为92.85%（26/28），2 组比较，差异有统计学
意义（P <0.05）。2 组VAS 评分、Lysholm 评分，IL-1β、TNF-α 水平治疗前后组内比较及治疗后组间比较，
差异均有统计学意义（P <0.05）。结论：正清风痛宁穴位注射治疗滑膜肿胀型KOA 寒湿痹阻证疗效显著，可
抗炎镇痛、改善关节功能，值得临床推广。 |

| Clinical effect of acupoint injection of Zhengqing Fengtongning in treatment of knee osteoarthritischaracterized by synovial swelling and cold-damp obstruction: An analysis of 28 cases |

| Objective: To investigate the clinical effect of acupoint injection of Zhengqing Fengtongning in the treatment
of knee osteoarthritis （KOA） characterized by synovial swelling and cold-dampness obstruction. Methods: A total
of 60 patients with KOA characterized by synovial swelling and cold-dampness obstruction were enrolled and randomly
divided into treatment group and control group, with 30 patients in each group. The patients in the control group received
oral administration of diacerein and celecoxib, while those in the treatment group received acupoint injection of Zhengqing
Fengtongning in addition to the treatment in the control group, and the course of treatment was 4 weeks for both groups. Two
patients dropped out from each group during treatment. The two groups were compared in terms of Visual Analogue Scale （VAS） score, Lysholm Knee Scoring Scale score, and the levels of interleukin-1β （IL-1β） and tumor necrosis factor-
α （TNF-α） in knee joint fluid before treatment and at 1, 2, and 4 weeks after treatment, and clinical outcome was
assessed. Results: There was a significant difference in overall response rate between the treatment group and the control
group ［100% （28/28） vs 92.86% （26/28）, P < 0.05］. After treatment, both groups had significant changes in VAS
score, Lysholm score, and the levels of IL-1β and TNF-α, and there were significant differences in these indicators between
the two groups after treatment （P < 0.05）. Conclusion: Acupoint injection of Zhengqing Fengtongning has a marked
clinical effect in the treatment of KOA characterized by synovial swelling and cold-dampness obstruction and can reduce
inflammation, alleviate pain, and improve joint function, and therefore, it holds promise for clinical application. |
