| 目的:采用 Meta 分析的方法系统评价中医药对肺结节患者炎症因子、免疫功能的影响。方法:计算机检索中国知网(China National Knowledge Infrastructure, CNKI) 、万方数据(WANFANG DATA)、中国生物医学文献服务系统(Chinese Biomedical Literature Service System, SinoMed) 、维普中文科技期刊数据库(VIP Information,VIP) 、PubMed 数据库中有关中医药治疗肺结节的随机对照试验(randomized controlled trial,RCT)。由 2 名研究人员根据纳入、排除标准独立筛选文献、提取数据,并采用Cochrane 偏倚风险评估工具评价研究质量,使用 Review Manager 5.3软件进行 Meta 分析。结果:本研究共纳入 14项RCTs,涉及患者1 485 例。Meta 分析结果显示,与对照组相比,中医药干预可显著缩小结节直径[MD=-1.78,95% CI(-2.00,-1.55),P<0.000 01],降低促炎因子白细胞介素-6 [MD=-5.78,95%CI(-8.77,-2.79),P=0.000 2]、肿瘤坏死因子-α [MD=-5.40,95%CI(-7.97,-2.82),P<0.000 1]的含量,提高CD4+ T细胞计数 [MD=5.36,95%CI(3.64,7.08),P<0.000 01] 和CD4+ /CD8+ 比值 [MD=0.43,95%CI(0.37,0.49),P<0.000 01] ,并降低CD8+ T细胞计数 [MD=-5.10,95% CI (-6.41,-3.78),P<0.000 01] 。结论:中医药可通过调节炎症免疫平衡及缩小结节直径发挥治疗肺结节的作用。 |

| Efficacy of traditional Chinese medicine in treatment of pulmonary nodules and its regulatory effect on inflammation and immune function: A meta-analysis |

| Objective: To systematically evaluate the effect of traditional Chinese medicine on inflammatory factors and immune function in patients with pulmonary nodules. Methods: CNKI,Wanfang Data,SinoMed,VIP,and PubMed were searched for randomized controlled trials (RCTs) on traditional Chinese medicine treatment of pulmonary nodules. Two researchers performed literature screening and data extraction independently according to the inclusion and exclusion criteria,and the Cochrane Collaboration’s tool for assessing risk of bias was used to assess the quality of studies. Review Manager 5.3 was used to perform the meta-analysis. Results: A total of 14 RCTs were included in this study,involving 1485 patients. The meta-analysis showed that compared with the control group,traditional Chinese medicine intervention significantly reduced the diameter of nodules (mean difference [MD] = -1.78,95% confidence interval [CI]: -2.00 to -1.55,P < 0.000 01) and the levels of the proinflammatory factors interleukin-6 (MD = -5.78,95% CI: -8.77 to -2.79,P = 0.000 2) and tumor necrosis factor-α (MD = -5.40,95% CI: -7.97 to -2.82,P < 0.000 1),increased the number of CD4+ T cells (MD = 5.36,95% CI: 3.64-7.08,P < 0.000 01) and CD4+/CD8+ ratio (MD = 0.43,95% CI: 0.37-0.49,P < 0.000 01),and reduced the number of CD8+ T cells (MD = -5.10,95% CI: -6.41 to -3.78,P < 0.000 01). Conclusion: Traditional Chinese medicine exerts a therapeutic effect on pulmonary nodules by regulating inflammation-immune balance and reducing the diameter of nodules. |
