| 目的:观察温针灸对膝骨关节炎(Knee Osteoarthritis,KOA)模型兔软骨下骨Fos原癌基因(Fos proto-oncogene,c-Fos)与Jun原癌基因(Jun proto-oncogene,c-Jun)mRNA表达的影响,探讨温针灸治疗KOA的可能作用机制。方法:选取18只普通级新西兰兔(雌雄各半),将其随机分为正常组、模型组和温针灸组,每组各6只,模型组与温针灸组采用4%木瓜蛋白酶关节腔注射法建立KOA模型。造模结束后开始干预,温针灸组取左侧内膝眼、犊鼻穴,行温针灸干预,20 min/次,隔天1次,连续干预4周;正常组和模型组仅固定、不干预。实验期间观察兔的一般情况;干预结束后,取左侧膝关节,使用番红O-固绿染色观察各组软骨及软骨下骨形态结构;实时荧光定量PCR(quantitative real-time PCR,qPCR)检测各组软骨下骨组织中c-Fos、c-Jun mRNA的表达情况。结果:与正常组比较,模型组兔一般情况较差,膝关节软骨组织表面粗糙,结构完整性被破坏,潮线模糊,骨小梁变细,稀疏,连续性中断,骨小梁表面积及表面积百分比降低(P<0.01),软骨下骨组织c-Fos、c-Jun mRNA相对表达量显著升高(P<0.01)。与模型组比较,温针灸组兔一般情况尚可,膝关节软骨及软骨下骨病理学表现有明显改善,潮线清晰,骨小梁增粗、增多,骨小梁表面积及表面积百分比升高(P<0.01);软骨下骨组织c-Fos、c-Jun mRNA相对表达量显著降低(P<0.01)。结论:温针灸可能通过下调c-Fos、c-Jun mRNA的表达,抑制破骨细胞的活性,从而减少软骨下骨吸收,调节软骨下骨重塑平衡,减轻软骨下骨损伤,进而延缓KOA的发展。 |

| Effect of warming acupuncture-moxibustion on the mRNA expression of Fos proto-oncogene and Jun proto-oncogene in subchondral bone of rabbits with knee osteoarthritis |

| Objective:To investigate the effect of warming acupuncture-moxibustion on the mRNA expression of Fos proto-oncogene (c-Fos) and Jun proto-oncogene (c-Jun) in subchondral bone of rabbits with knee osteoarthritis (KOA),as well as the possible mechanism of action of warming acupuncture-moxibustion in the treatment of KOA.Methods:A total of 18 ordinary New Zealand rabbits (9 male rabbits and 9 female rabbits) were selected and randomly divided into normal group,model group,and warming acupuncture-moxibustion group,with 6 rabbits in each group.The rabbits in the model group and the warming acupuncture-moxibustion group were given intraarticular injection of 4% papain to establish a model of KOA.Intervention was started after modeling,and the rabbits in the warming acupuncture-moxibustion group were given warming acupuncture-moxibustion at left Neixiyan and Dubi points for 20 minutes each time,once every other day for 4 consecutive weeks;The rabbits in the normal group and the model group were only fixed without intervention.The general conditions of the rabbits were observed during the experiment;after intervention,the left knee joint was collected,and safranin O-fast green staining was used to observe the morphological structure of cartilage and subchondral bone in each group;quantitative real-time PCR was used to measure the mRNA expression levels of c-Fos and c-Jun in subchondral bone.Results:Compared with the normal group,the model group had worse general conditions,with a rough surface of knee joint cartilage tissue,a damaged structure,and an unclear tidal line,and the bone trabecula became thinner,sparse,and fractured,with significant reductions in the surface area of bone trabecula and the percentage of surface area (P < 0.01);there were significant increases in the relative mRNA expression levels of c-Fos and c-Jun in subchondral bone (P < 0.01).Compared with the model group,the warming acupuncture-moxibustion group had fairly good general conditions and significant improvements in the pathological manifestations of knee joint cartilage and subchondral bone,with a clear tidal line,thickening of bone trabecula,and significant increases in the surface area of bone trabecula and the percentage of surface area (P < 0.01),as well as significant reductions in the relative mRNA expression levels of c-Fos and c-Jun in subchondral bone (P < 0.01).Conclusion:Warming acupuncture-moxibustion can downregulate the mRNA expression levels of c-Fos and c-Jun,inhibit the activity of osteoclasts,reduce the absorption of subchondral bone,regulate the balance of subchondral bone remodeling,and alleviate the damage of subchondral bone,thereby delaying the development of KOA. |
