| 目的：观察腰大肌抗阻训练结合针刺对改善脑卒中后偏瘫步态的临床疗效。方法：将60 例脑卒中后偏瘫患者随机分为治疗组和对照组，每组各30 例。对照组单用针刺疗法，治疗组在对照组治疗的基础上配合腰大肌抗阻训练，2组均每天治疗1次，每周治疗5 d，以21 d为1个疗程，连续治疗3个疗程。观察2组治疗前后功能性步行（Functional ambulation category，FAC）评分、伯格平衡量表（Berg balance scale，BBS）评分、梅尔氏运动功能评定量表（Fugl-Meyer assessment，FMA）评分、日常生活活动（activity of daily living，ADL）评分。结果：总有效率治疗组为90.00%（27/30），高于对照组的76.67%（23/30），差异有统计学意义（P＜0.05）。2组FAC、BBS、FMA及ADL评分均有改善，且治疗组改善幅度大于对照组（P＜0.05）。结论：腰大肌抗阻训练结合针灸治疗脑卒中后偏瘫步态，可以产生协同效果，有助于改善脑卒中后偏瘫患者的功能性活动能力、平衡能力、运动功能及日常生活能力。 |

| Clinical effect of resistance training of psoas major combined with acupuncture in treatment of post-stroke hemiplegic gait：An analysis of 30 cases |

| Objective：To investigate the clinical effect of resistance training of psoas major combined with acupuncture in improving post-stroke hemiplegic gait.Methods：A total of 60 patients with post-stroke hemiplegia were randomly divided into treatment group and control group，with 30 patients in each group.The patients in the control group were given acupuncture alone，and those in the treatment group were given resistance training of psoas major in addition to the treatment in the control group，once a day for 5 days a week.Each course of treatment was 21 days，and the two groups were treated for 3 consecutive courses.The two groups were observed in terms of Functional ambulation category(FAC) score，Berg balance scale(BBS) score，Fugl-Meyer assessment (FMA) score，and activity of daily living (ADL) score before and after treatment.Results：The treatment group had a significantly higher overall response rate than the control group ［90.00%(27/30)vs76.67%(23/30)，P＜0.05］.After treatment，both groups had improvements in FAC，BBS，FMA，and ADL scores，and the treatment group had significantly greater improvements than the control group (P＜0.05).Conclusion：Resistance training of psoas major combined with acupuncture has a synergistic effect in the treatment of post-stroke hemiplegic gait and can improve functional ambulation ability，balance ability，motor function，and activities of daily living. |
