| 目的：观察芒针降逆针刺法联合足三里穴位注射治疗中风后顽固性呃逆的临床疗效。方法：将68例中风后顽固性呃逆患者随机分为治疗组和对照组，每组各34例。对照组予以西医常规治疗联合盐酸甲氧氯普胺注射液穴位注射治疗，治疗组在对照组的基础上加用芒针降逆针刺法治疗，2组均连续治疗2周，于治疗前后观察2组呃逆发生频率、呃逆持续时间、日常生活质量评分表（activities of daily living，ADL）评分、汉密尔顿焦虑量表（hamilton anxiety scale，HAMA）评分、汉密尔顿抑郁量表（hamilton depression scale，HAMD）评分，并评价临床疗效。结果：总有效率治疗组为94.12%（32/34），对照组为85.29%（29/34），差异有统计学意义 （P<0.05）。2组呃逆发生频率、呃逆持续时间、ADL评分、HAMA评分和HAMD评分治疗前后组内比较及治疗后组间比较，差异均有统计学意义（P<0.05）。结论：芒针降逆针刺法联合足三里穴位注射可有效减轻中风后顽固性呃逆患者的临床症状，并改善患者生活质量及心理状态，值得临床推广。 |

| Clinical effect of elongated needle acupuncture for downbearing counterflow in treatment of intractable hiccups after stroke: An analysis of 34 cases |

| Objective： To investigate the clinical effect of elongated needle acupuncture for downbearing counterflow combined with acupoint injection at Zusanli in the treatment of intractable hiccups after stroke. Methods： A total of 68 patients with intractable hiccups after stroke were randomly divided into treatment group and control group, with 34 patients in each group. The patients in the control group received conventional Western medicine treatment combined with acupoint injection of metoclopramide hydrochloride injection, while those in the treatment group received elongated needle acupuncture for downbearing counterflow in addition to the treatment in the control group, and both groups were treated for 2 consecutive weeks. The two groups were observed in terms of the frequency of hiccups, the duration of hiccups, Activities of Daily Living （ADL） score, Hamilton Anxiety Scale （HAMA） score, Hamilton Depression Scale （HAMD） score before and after treatment, and clinical outcome was assessed for both groups. Results： There was a significant difference in overall response rate between the treatment group and the control group ［94.12% （32/34） vs 85.29% （29/34）, P < 0.05］. After treatment, both groups had significant changes in the frequency of hiccups, the duration of hiccups, ADL score, HAMA score, and HAMD score, and there were significant differences in these indicators between the two groups after treatment （P < 0.05）. Conclusion： For patients with intractable hiccups after stroke, elongated needle acupuncture for downbearing counterflow combined with acupoint injection at Zusanli can effectively alleviate clinical symptoms and improve quality of life and psychological state, and therefore, it holds promise for clinical application. |
