| 目的：观察归脾养心丸治疗射血分数保留慢性心力衰竭（chronic heart failure with preserved ejection fraction， HFpEF）伴贫血的临床价值。 方法：将60例HFpEF伴贫血气血两虚血瘀证患者随机分成治疗组与对照组，每组各30例。对照组予以西医常规治疗，治疗组在对照组基础上加用归脾养心丸治疗，2组均连续治疗4周。于治疗前后观察2组中医证候积分、6分钟步行距离（6-minute walk distance，6MWD）、 贫血指标［红细胞计数（red blood cell count，RBC）、血红蛋白（hemoglobin， Hb）、 红细胞压积（hematocrit， HCT）、铁蛋白（ferritin content， SF）］及N末端脑钠肽前体（N-terminal pro-brain natriuretic peptide， NT-proBNP）水平变化，并评价临床疗效、纽约心脏病学会（New York Heart Association， NYHA）心功能分级疗效。结果：总有效率治疗组为86.67%（26/30），对照组为66.67%（20/30），2组比较，差异有统计学意义（P<0.05）。2组中医证候积分、6MWD、贫血相关指标（RBC、Hb、HCT）和NT-proBNP水平治疗前后组内比较、治疗后组间比较及心功能分级疗效治疗后组间比较，差异均有统计学意义（P<0.05或<0.01）；而SF水平仅治疗前后组内比较，差异有统计学意义（P<0.05）。结论：归脾养心丸联合西医常规治疗可改善HFpEF伴贫血气血两虚血瘀证患者的临床症状，值得临床推广。 |

| Clinical effect of Guipi Yangxin pills in treatment of chronic heart failure with preserved ejection fraction accompanied by anemia: An analysis of 30 cases |

| Objective： To investigate the clinical value of Guipi Yangxin pills in the treatment of chronic heart failure with preserved ejection fraction （HFpEF） accompanied by anemia. Methods： A total of 60 patients with HFpEF and anemia of Qi-blood deficiency and blood stasis syndrome were randomly divided into treatment group and control group, with 30 patients in each group. The patients in the control group received conventional Western medicine treatment, while those in the treatment group received Guipi Yangxin pills in addition to the treatment in the control group, and both groups were treated for 4 consecutive weeks. The two groups were observed in terms of traditional Chinese medicine （TCM） syndrome score, 6-minute walk distance （6MWD）, anemia indicators （red blood cell count ［RBC］, hemoglobin ［Hb］, hematocrit ［HCT］, and serum ferritin ［SF］）, and N-terminal pro-brain natriuretic peptide （NT-proBNP） before and after treatment, and clinical outcome , the treatment outcome of New York Heart Association （NYHA） cardiac functional grading were assessed for both groups. Results： There was a significant difference in overall response rate between the treatment group and the control group ［86.67% （26/30） vs 66.67% （20/30）, P < 0.05］. After treatment, both groups had significant changes in TCM syndrome score, 6MWD, anemia-related indicators （RBC, Hb, and HCT）, and NT-proBNP, with significant differences between the two groups after treatment, and there was a significant difference in the treatment outcome of cardiac functional grading between the two groups after treatment （P < 0.05 or P < 0.01）; both groups had a significant change in SF level after treatment （P < 0.05）. Conclusion： Guipi Yangxin pills combined with conventional Western medicine treatment can improve the clinical symptoms of patients with HFpEF and anemia of Qi-blood deficiency and blood stasis syndrome and thus holds promise for clinical application. |
