| 目的：探讨补阳还五汤治疗Ⅲ～Ⅳ期糖尿病肾病气虚血瘀证的临床疗效。方法：将90 例Ⅲ～Ⅳ期糖尿病肾病气虚血瘀证患者随机分为治疗组和对照组，每组各45 例。对照组予西医常规治疗及饮食、运动、糖尿病健康宣教；治疗组在对照组基础上加用补阳还五汤治疗，疗程均为8周。比较2组综合疗效及治疗前后中医证候积分、血糖指标［空腹血糖（FPG）、餐后2h血糖（2hPG）、糖化血红蛋白（HbA1c）］、血脂指标［三酰甘油（TG）、胆固醇（TC）、低密度脂蛋白（LDL-C）］、肾功能相关指标［血肌酐（SCr）、尿素氮（BUN）、尿白蛋白排泄率（UAER）］。结果：总有效率治疗组为93.33%（42/45），高于对照组的75.56%（34/45），差异有统计学意义（P＜0.05）；2组中医证候积分及血糖、血脂、肾功能相关指标均较治疗前降低，且治疗组下降幅度大于对照组，差异均有统计学意义（P＜0.05）。结论：补阳还五汤能改善Ⅲ～Ⅳ期糖尿病肾病气虚血瘀证患者的中医证候，加快糖、脂代谢，保护肾功能，从而达到延缓病情进展的目的。 |

| Clinical effect of Buyang Huanwu decoction in treatment of stage Ⅲ～Ⅳdiabetic nephropathy with Qi deficiency and blood stasis syndrome |

| Objective：To investigate the clinical effect of Buyang Huanwu decoction in the treatment of stage Ⅲ～Ⅳ diabetic nephropathy with Qi deficiency and blood stasis syndrome.Methods：A total of 90 patients with stage Ⅲ～Ⅳ diabetic nephropathy with Qi deficiency and blood stasis syndrome were randomly divided into treatment group and control group，with 45 patients in each group.The patients in the control group were given conventional Western medicine treatment and education on diet，exercise，and diabetes，while those in the treatment group were given Buyang Huanwu decoction in addition to the treatment in the control group，and the course of treatment was 8 weeks for both groups.The two groups were compared in terms of overall response and related indicators before and after treatment，including traditional Chinese medicine (TCM) syndrome score，blood glucose parameters (fasting plasma glucose，2-hour postprandial blood glucose，and glycosylated hemoglobin)，blood lipid parameters (triglyceride，cholesterol，and low-density lipoprotein)，and renal function parameters (serum creatinine，blood urea nitrogen，and urinary albumin excretion rate).Results：The treatment group had a significantly higher overall response rate than the control group ［93.33% (42/45) vs 75.56% (34/45)，P＜0.05］.After treatment，both groups had significant reductions in TCM syndrome score，blood glucose parameters，blood lipid parameters，and renal function parameters，and the treatment group had significantly greater reductions than the control group (P＜0.05).Conclusion：In patients with stage Ⅲ～Ⅳ diabetic nephropathy with Qi deficiency and blood stasis syndrome，Buyang Huanwu decoction can improve TCM syndrome，accelerate glucose and fat metabolism，and protect renal function，thereby delaying disease progression. |
