| 目的：观察祛风除痒汤治疗原发性肛门瘙痒症的临床疗效及作用机制。方法：将76例原发性肛门瘙痒症患者随机分为治疗组46例、对照组30例，治疗组采用祛风除痒汤坐浴治疗，对照组采用高锰酸钾粉坐浴治疗，2组均以7d为1个疗程，共治疗2个疗程。观察2组肛门瘙痒及皮损积分变化，检测治疗后患者血浆β-内啡肽、亮氨酸脑啡肽的浓度变化。结果：总有效率治疗组为86.96%，对照组为76.67%，2组比较，差异有统计学意义（P<0.05）；2组治疗后肛门瘙痒及皮损面积总积分均较治疗前下降，治疗组改善优于对照组，差异均有统计学意义（P<0.05）；治疗组治疗后β-内啡肽水平明显高于对照组（P<0.05），亮氨酸脑啡肽水平差异无统计学意义（P＞0.05）。结论：祛风除痒汤治疗原发性肛门瘙痒症可取得良好的效果，其作用机制可能与降低血浆β-内啡肽的浓度、减轻中枢性致痒有关。 |

| Clinical effect of Qufeng Chuyang decoction in treatment of primary peritus ani and its mechanism of action |

| Objective：To investigate the clinical effect of Qufeng Chuyang decoction in the treatment of primary peritus ani and its mechanism of action.Methods：A total of 76 patients with primary peritus ani were randomly divided into treatment group with 46 patients and control group with 30 patients.The patients in the treatment group were given sitz bath with Qufeng Chuyang decoction，and those in the control group were given sitz bath with potassium permanganate powder.Each course of treatment was 7 days，and both groups were treated for two courses.The two groups were observed in terms of the changes in the scores of peritus ani and skin lesion，and the plasma levels of β-endorphin and leucine encephalin were measured.Results：There was a significant difference in overall response rate between the treatment group and the control group（86.96% vs 76.67%，P<0.05).Both groups had significant reductions in the scores of peritus ani and skin lesion after treatment，and the treatment group had significantly greater improvements than the control group（P<0.05).After treatment，the treatment group had a significantly higher level of β-endorphin than the control group（P<0.05)，while there was no significant difference in the level of leucine encephalin between the two groups（P>0.05).Conclusion：Qufeng Chuyang decoction has a good clinical effect in the treatment of primary peritus ani，possibly by reducing the plasma concentration of β-endorphin and alleviating central pruritus. |
